The Signs of Aging
Some of the changes associated with an aging face are related to volume loss and structural changes. There is also a loss of skin elasticity—the skin’s ability to bounce back. Loss of elasticity is like a rubber band that has been used too much or left in a drawer too long—it stretches but stays stretched, unable to return to its original shape. In that state, it becomes useless as a rubber band. The same thing happens with your skin. These two factors—volume loss and structural changes, along with reduced elasticity—contribute to the changes you are seeing.
Volume loss in the upper face—such as the loss and downward movement of fat pads, as well as subtle bone loss in the upper cheek—causes the skin to drop. With decreased skin elasticity, it no longer bounces back or holds itself up. In the lower face, there is a loss of soft tissue over the chin and bone loss in the lower jaw (mandible). This leads to a shortened jawline, a weaker chin, shadowing of the chin, reduced lip volume and projection, and the formation of jowls. Volume loss in the temples results in hollowing and contributes to an older appearance. Weight loss or a naturally low body weight can accentuate these changes.
Maybe none of these changes are occurring because you are still quite young. However, you may still be troubled by facial asymmetry or inherently small features—such as a short jawline, small chin, thin cheeks or temples, or deep-set eyes. Yes, it’s true! Fillers can renew and tighten your skin—this is true regenerative medicine. These fillers stimulate your skin to renew its structure by producing collagen, regenerating elastin, and rebuilding the extracellular matrix, delivering skin renewal at multiple levels through biostimulation. How does this happen?
We mainly use what are called biostimulatory fillers to initiate this skin renewal process. There are two predominant biostimulatory fillers: Sculptra (poly-L-lactic acid) and Radiesse (calcium hydroxylapatite). Hyaluronic acid fillers can also have a biostimulatory effect, but are primarily used to restore volume, create lift, hydrate, and soften lines.
When a biostimulatory filler is injected into the dermal layer of the skin, the skin’s response to the filler produces the rejuvenating effect. The particles of the filler attract the attention of skin cells called fibroblasts. Without stimulation, these fibroblasts are dormant. They are what we call senescent cells, meaning they are present in the tissue but not performing their function and may even contribute to accelerated aging. When awakened by particles of biostimulatory filler, these aging fibroblasts stretch out, attach to the particles, and start functioning again. Their function is to produce collagen, elastin, and extracellular matrix—improving the structure of our skin.

Preparing for Your Charlotte, NC, Skin Filler Appointment
It is ideal to follow these instructions before your appointment:
- Avoid aspirin and ibuprofen for one week before your appointment to minimize bruising
- Don’t consume alcohol or caffeine for 24 hours before your appointment, as it can increase bleeding and bruising risk
- Use a gentle cleanser the morning before your appointment, and don’t wear makeup
- Your comfort is our priority. Discuss with us if you remain anxious about your appointment. You may benefit from an oral medicine to help you relax. We ask that you not drive if one of these medicines is given.
What to Expect During Your Skin Filler Appointment
Prior to your injection session, you will be asked to remove makeup and earrings. Photos will be taken, and you will be asked to sign a consent form. This form is available on our TouchMD app, allowing you to sign it ahead of your appointment. If you have any questions, they can be addressed before we begin.
We will review your photos, discuss any treatment plan, and go over any previous injection treatments you have had. Then, we will evaluate you in person and discuss your goals. After this, the filler will be prepared.
Your skin will be cleansed with an antiseptic cleanser. Tiny injections of numbing medication will be placed in the treatment area to allow for filler injection via a cannula. A cannula is a fine, blunt-tipped hollow tube used for injections. Because it is blunt-tipped—not sharp like a needle—the risks of bruising and injecting into a blood vessel are minimized. Multiple filler placement areas can be reached from a single insertion point. At the end of the injection session, the treated areas may be gently massaged.
We often blend the filler with a small amount of lidocaine or another numbing agent to ensure minimal discomfort as the injections proceed, since the area becomes quite numb. This numbing effect typically lasts about an hour. Because of this, it’s often comfortable to have a laser or radiofrequency treatment immediately following filler injections.
Keeping You Safe During Your Charlotte, NC, Filler Injections
Serious side effects from filler injections are primarily related to inadvertent injection into an artery. Arteries supply blood to the skin, so if an artery becomes blocked by a bolus of filler, the skin may be compromised. Large areas can be affected, potentially leading to necrosis, or tissue death, due to a lack of blood supply. This is a medical emergency and can later result in wound healing issues or require surgical correction.
If this occurs, we will immediately attempt to dissolve the filler in the blood vessel by locating it with ultrasound and injecting a dissolving agent, hyaluronidase, into the area. We can also inject hyaluronidase blindly around the area, as it is safe for the surrounding tissues.
To minimize the risk of injecting into an artery, the injectors at Charlotte Skin & Laser follow these protocols:
- Use a blunt-tipped cannula
- Use slow, tiny bolus injections
- Keep the cannula tip moving at all times
During your treatment, it’s important to focus on your comfort and stay relaxed. Anxiety and muscle tension can cause facial blood vessels to enlarge, increasing the risk of bleeding, bruising, and vascular complications. Please be open and honest with us about your anxiety, pain tolerance, and comfort level throughout the treatment.
