For patients who are seeing the early stages of age-related volume loss, a “liquid facelift” that combines injectable fillers with a neuromodulator like Botox can restore a more youthful appearance, possibly delaying the need for more invasive plastic surgery procedures like facial fat transfer.

That said, dermal fillers will not provide the same level of facial rejuvenation as a cosmetic surgery like a facelift. Results are far more subtle and temporary.

Fillers are gel-like synthetic products made up of various chemicals designed to mimic chemicals that are naturally present in the body. Some are composed of hyaluronic acid (HA), while others are made of calcium hydroxylapatite (CaHA) or poly-L-lactic acid (PLLA).

These products are injected into the targeted area, where they create either immediate or gradual volume. Fillers slowly and safely break down over the course of months or in some cases years. Patients can return for maintenance treatments to preserve their filler results.

Hyaluronic acid fillers increase the supply of hyaluronic acid in the skin. HA bonds with water molecules in the tissues to create new volume.

At the start of a treatment session, the intended injection sites are cleansed. They may be marked as a guide for the injector. Precise amounts of filler are then injected into the marked areas.

Many fillers contain lidocaine, a local anesthetic, providing numbing against any pain or discomfort. For fillers that do not contain lidocaine, local or topi- cal anesthesia can be administered before the injections are delivered.

For hyaluronic fillers, patients can see results immediately after the treatment. Radiesse’s results are also instantaneous. Sculptra’s results appear gradually and typically start to appear around 6 weeks after the treatment session.
The patient may experience minor bruising, redness, tenderness, or swelling in the treatment areas. The area may feel numb or sensitive. Aftercare instructions will be provided to the patient.
The results of HA filler injections can last anywhere from 6 months up to 2 years, depending on the specific filler product chosen. Results for stimulatory fillers can last 12 to 24 months and in some cases even longer.
Side effects — which tend to occur around the injection site — can be immediate, but usually clear within 7 to 14 days:

  • redness
  • swelling
  • pain
  • bruising
  • itching
  • rash
While there are several alternatives to fillers, most (like neuromodulators, CO2 skin resurfacing, and Venus Viva) are designed to treat shallow lines rather than moderate or harsh wrinkles. For volume restoration, alternatives are typically surgical in nature and are therefore much more invasive.
After the treatment session, the patient can immediately go right back to their normal activities without downtime or recovery time. The treatment area must be protected from trauma and sun exposure.
